cRPM shows potential reduced HF-related readmissions but requires iterative, in-depth analysis of alerting structure, communication pathways, and workflow logic to adapt and reconfigure protocols in a change management and organizational learning process. Our results suggest that a human-centered socio-technical approach, coupled with negotiated engagement and empowerment of frontline workers, is essential to scale the study to evaluate clinical and financial outcomes.
